In December 2019, we entered into an agreement with Align Senior Care. By doing so, we are able to offer Medical Advantage Special Needs Plans (SNP) to our residents. With an SNP, residents have access to traditional Medicare benefits plus added enhanced clinical care. Each member is also assigned a dedicated Nurse Practioner to see and help coordinate with their PCP’s.
