 As we strive to continue providing the highest level of care for our residents, we have entered into a partnership with Align Senior Care to offer a Medicare Advantage Special Needs Plan (SNP) to all of our residents. With Align Senior Care, residents can have all of the benefits of traditional Medicare coverage, plus added enhanced clinical care, comprehensive care planning, best practices, collaboration, and supplement benefits that go beyond Medicare, such as podiatry, vision, and dental. 

Each member also receives a dedicated Nurse Practitioner that will see members and help coordinate with their PCPs.

 WHAT IS A SPECIAL NEEDS PLAN (SNP)? 

 Medicare SNPs are a type of Medicare Advantage Plan (like an HMO). Medicare SNPs are specifically designed for Medicare Beneficiaries who may be at higher risk and who reside in specific settings such as long-term care, assisted or independent living. SNPs can also focus on beneficiaries with specific medical diagnosis or chronic condition. Medicare SNPs tailor their benefits, provider choices, and drug formularies to best meet the specific needs of the groups they serve. 

Chronic-Condition Special Needs Plans: 

These plans serve beneficiaries with certain severe or disabling chronic conditions, such as Alzheimer’s, cancer, or chronic health failure.

Institutional Special Needs Plans: 

These plans serve those living in Skilled Nursing, Assisted Living, or Independent Living.
