When is rehab needed?

Our Rehabilitation Process
- Referral – Any physician (primary care, emergency room, skilled nursing, etc.) can make a referral and order therapy at any setting, including your home, assisted living facility, or skilled nursing facility.
- Initial Evaluation – Our team evaluates your current abilities, needs, and personal goals.
- Personalized Therapy Plan – We design a program with specific exercises, activities, and strategies to help you reach your goals. The intensity of therapy services, how often, and how long they occur are based on each individual’s needs.
- Daily Therapy Sessions – These may include physical, occupational, and/or speech therapy depending on your needs.
- Progress Check-Ins – Your plan evolves as you improve, keeping you on track for the best outcome possible.
- Discharge Planning – Before you leave, we ensure you have a plan in place for continued success at home or in another level of care. Our discharge planner can help coordinate therapy services as you transition through the continuum of care and will verify that benefits will continue to be covered.
Post Rehab: What’s next?
Many people return home after completing rehab, sometimes with added support from in-home care services through our partners at Atrio Home Care. Others may find that they benefit from a transition to Assisted Living, where they can continue to receive support in a vibrant, social environment.
