Advancing Connected Care for Adults with Disabilities in West Michigan

Three West Michigan nonprofits—including Holland Home and Pine Rest Christian Mental Health Services—have partnered to launch a coordinated care network designed to better serve adults with intellectual and developmental disabilities.

The collaboration focuses on improving transitions between services and reducing fragmentation in care. By aligning resources and expertise across organizations, the network aims to create a more seamless experience for individuals and families—particularly as needs evolve over time.

This model reflects a growing emphasis on integrated, person-centered care, helping ensure that individuals don’t fall through gaps between providers and can access the right level of support at the right time. Pine Rest Christian Mental Health Services, one of the region’s leading behavioral health providers, brings clinical expertise to the partnership, complementing residential and community-based services.
