Our Life Plan Communities
Breton Woods and Raybrook Life Plan Communities provide a home to over 1,300 residents, each offering a full continuum of specialty care services and a Lifecare Promise that frees residents from financial worries should their needs outlast their resources.
To complement our residential living and care services, Holland Home also supports a broad network of care partners and affiliates to address the varying needs of aging adults living throughout Grand Rapids and West Michigan.

Faith Hospice
Offering in-patient services at their Trillium Woods facility and in-home services wherever a patient calls home, Faith Hospice brings comfort, support, and compassion to the end-of-life journey. From medical care to pain management and bereavement counseling, Faith Hospice focuses on improving the quality of life for their patients, caring for thier loved ones, and educating the community.
Reliance Community Care Partners
Reliance has been a resource for seniors, caregivers, and others with long-term care needs since 1979. Through various activities, case managers coordinate services that help with daily life and assure each individual has the gentle and dignified care they need.
Dementia Institute
The Dementia Institute provides training that empowers caregivers, care that supports autonomy, and research that promotes excellence. We are here to serve with empathy and expertise so that those living with dementia and the community that surrounds them can have more meaningful interactions and an enhanced quality of life.
Trillium Institute
Trillium Institute is fully committed to improving and advancing medical care for those with life-limiting and terminal illnesses. Recognizing the growing need for quality hospice and palliative care, Trillium Institute exists to raise awareness of the value of these services and provide training to medical professionals, ensuring the next generation of healthcare providers is well equipped to meet the escalating demands.
Collaborative Care Partners
Atrio Home Care
Atrio Home Care meets the different needs of aging adults by uniting two distinct providers: Atrio Help at Home and Atrio Home Health. Their Help at Home Private Duty Aides assist seniors with daily tasks and provide much-needed companionship. The Atrio Home Health team provides more complex clinical care services for seniors recovering from surgery, illness, or stroke.
Care Resources
A community-based program for people 55+ that promotes healthy, independent living. This Program of All-Inclusive Care for the Elderly (PACE) allows older adults to continue receiving the managed care and support they need in a day center or home setting.
TANDEM365
A collaborative venture of health care professionals that enables older adults to remain in their homes by helping them navigate the complexities of healthcare. Through customized care coordinated by their team, participants in TANDEM365 have access to the best services and support available with a single point of contact.
