Telehealth: Home healthcare for the 21st century
You bank online, read the news online, make travel plans online — why shouldn’t healthcare for you or your loved one be as advanced as you are?
Telehealth is the answer.
HomeCare of Holland Home provides daily telehealth monitoring of qualified clients with cardiac, pulmonary, and other diagnoses requiring assistance with disease management. Telehealth allows for remote monitoring of vital signs, including blood pressure, weight, heart rate, pulse oximetry, and blood sugars, and allows for preventative and proactive care, enabling nurses and physicians to intervene early with changes to a patient’s health.
But don’t just take our word for how great it is — check out the eightWest video feature about telehealth, which includes an interview with a patient benefitting from telehealth services.
Who benefits from telehealth monitoring?
Everyone in the caregiving process benefits from telehealth monitoring:
- Patients are empowered to take control of their disease, and feel secure that their health conditions are being monitored daily and that changes to their health profile will be detected and corrected quickly.
- Families and caregivers have the peace of mind that their loved ones are receiving daily monitoring of their symptoms. And because telehealth has been shown to reduce rehospitalization by 50%, patients are able to maintain their independence.
- Physicians coordinate with our home health nurses to determine which patients will benefit from telehealth monitoring and set notification parameters specific to each patient.
Easy as 1, 2, 3
- Patients help to manage their health conditions by using the interactive system each day, in their homes, to monitor vital signs, symptoms, and take a daily health check.
- The patient’s daily health information is automatically sent securely over a phone line to the patient’s medical team in less than 30 seconds.
- The patient’s health data is analyzed and their physician is notified if readings or symptoms may indicate a need for change in treatment plan.
How does it work?
Patients receive a device in their home that will check their pulse, blood pressure, oxygen saturation, and weight. Monitoring is done seven days a week and only takes a few minutes a day. The data is transmitted securely by telephone without the patient needing to do anything further.
How much does such an advanced service cost?
That’s the best part. There is no additional cost for telehealth monitoring for Medicare patients under the care of HomeCare of Holland Home.
