Care coordination
Most people don't know where to turn when they need help supporting themselves or their loved one with remaining in their home.
Care coordination provides a social worker and/or licensed nurse to assist you with developing a person-centered care plan that identifies your needs and develops a plan for you to reach your maximum level of independence. Our staff will assist you with selecting services and providers to meet your needs.
Care coordination may include:
- Home-delivered meals set up
- In-home respite care planning
- Short-term out-of-home respite care arrangements
- Personal emergency response system activation
- Automated medication device activation
- Coordination with medical professionals
