Administrative Assistant 1 - Hospice (part-time)

Job Description

Provides clerical support to assigned function(s).  Duties include, but are not limited to, answering calls and forwarding to appropriate staff members, typing, data entry of information. Interacts with all levels of staff, residents/patients/clients, families on the phone offering information and assistance or directing the person to the appropriate department.  Organize patient files and data entry.

 Top candidates will have proven administrative assistant skills, very detail oriented, computer literate,  and able to work with multiple interruptions.

Job Details

This position works Monday through Friday from 10am - 2pm.  It is 20 hours per week and considered part-time for Faith Hospice.

Biller/Coder

Job Summary

Responsible for coding, billing and revenue cycle management. Ensure appropriate coding for reimbursement. Responsible for monitoring compliance with the coding and billing regulations for behavioral health services. Monitor 3rd party billing, posting payment to accounts, and identifying accounts requiring follow-up. Support the claims unit in processing of incoming claims, submitting 837 files to the State of Michigan, and providing resolution of any errors returned on the submission. Maintain required databases.

Job Requirements

Requires knowledge of accounts receivable processes. Billing and/or coding experience is preferred. Experience with the use of CPT4, ICD-9, DSM-4 and DSM-5 coding. Knowledge of governmental regulations relating to medical record documentation, coding and billing processes preferred. Working knowledge of Medicare, Medicaid, private insurance, worker's comp benefits and coverage criteria.

Center Manager (RN or MSW) - Care Resources

Job Summary

Manage the daily functions of the adult day health center in conformance with Care Resources policies, procedures, and standards and PACE regulations. Facilitate the PACE interdisciplinary team.

Job Qualifications

Requires licensed RN or MSW in the State of Michigan; requires a minimum of 3 years of experience in a health care setting and strongly value experience in health plan administration. Requires at least one year of supervisory related responsibilities.  All Care Resources staff must have at least one year of experience within their discipline of working with the frail or elderly population.

Job Description

Floor Care Specialist

Job Summary
Provides scheduled floor maintenance, corrective and care of corridor and other assigned areas of the facility in accordance with housekeeping procedures, schedules and standards or performance.

Job Qualifications
Ability to lift and move furniture (a minimum of 50 pounds), work in small areas, bend and kneel easily and ability to use a ladder.  Must be a self-motivated person and be able to work independently.  Preferred experience with industrial carpet cleaning equipment.

Grounds Technician

Job Description

Performs routine exterior maintenance to resident homes and other buildings on all 4 Holland Home campuses in the greater Grand Rapids area.

Requires knowledge normally acquired through a general high school diploma or equivalent. Requires 1-2 years of exterior maintenance experience.  Proof of Michigan driver's license, vehicle liability insurance, and reliable transportation are also required.

Helpers of Holland Home - Home Assistant

Responsibilities
Providing in home care for the elderly within the community through personal cares, light housekeeping, basic food prep, and transportation needs. Also includes assisting with ambulation and activities of daily living.

Skills required
Valid MI drivers license, reliable source of transportation, and current auto insurance. Experience working with the elderly is preferred but not required. Must have strong interpersonal skills and be organized and self motivated. Successful candidates have good computer skills and have access to email.

Other information
Prefer at least 4 hours of consecutive open availability or 8 hours of full shift availability.  Position will require 3 holidays a year and one week-end a month of on call responsibility.

Details
This position is at Helpers of Holland Home. It is a part time position during all shifts, and provides an average 15/20 hours per week may be more with individual availability.

Housekeeping

Job Summary

Job Summary

The Intake Coordinator evaluates telephone inquiries for Long Term Care Programs to determine potential program eligibility and schedule assessments.  Provides assessment, education and referral management as needed for program participants.

Job Qualifications

This position requires a licensed Master's prepared Social Worker (LLMSW or LMSW) or licensed Practical Nurse (LPN) in the State of Michigan as well as a minimum of 3 years of clinical care experience in a related field. Top candidates will also have a valid driver's license, valid motor vehicle insurance, and reliable transportation. Community case management experience is highly preferred as well as knowledge of community resources and completion of the Gerontology Certificate Program.

Job Details

This position is full-time; however, 24 or 32 hours per week may be considered. It is an office position, based at the HHS, Health Options Corporate Office in Grand Rapids.

LPN - Weekend On Call Rotation

The candidate would not be required to work every weekend, but would be added to a rotation with other Licensed Nurses and would fill in as back up as needed. Shift is primarily Friday 4:30 pm – 8:30 am the following Monday.   All after hours on call work is performed remotely.  This does not mean you must stay home.  You may be mobile, but you are required to respond within 30 minutes to all calls.

Physical Therapist - certified home care, full-time

Job Description

Develop and implement physical therapy treatment plans for assigned clients. This includes completing initial physical therapy evaluations and ongoing assessments to determine level of functioning, completing the OASIS documentation, and coordinates servcies in consultation with other home care staff and families.

Requires licensure as a physical therapist in the State of Michigan and a minimum of 1 year of experience as a physical therapist. Geriatric experience is highly valued.

Job Description

Tandem365 is seeking clinical case management staff who have the ability and skills to help our participants navigate through the health care system . This is a community based program, where the RN Navigator will be providing participant support, caregiver support, and assessment of needs in the home, doctor’s office, or a facility.

Job Qualifications

This position requires an RN license in the State of Michigan. The ideal candidate will be well organized, self-motivated, and driven to help develop a new program while providing in-home care, education, and continued coordination of care to their participants. Must have the ability to work well as a team as well as independently. Minimum of 2 years of acute care and/or certified home care experience required. Prefer someone with prior experience in community based services providing case management.

Job Summary

Job Responsibilities

Assess client’s health status, establish nursing diagnoses, develop and implement nursing care plan under the orders of the supervising physician. Maintain communications with clients, caregivers, interdisciplinary home care team members and physicians to enable the client to achieve the highest level of function possible.

Michigan licensure as an RN required. BSN preferred. Per federal mandates, minimum of 2 years experience in an in-patient or community mental health setting. Social Work (MSW) Health Care Navigator - HHS, Health Options

Job Description

Holland Home of Grand Rapids, Clark Retirement, Life EMS, Porter Hills, and Sunset Retirement Communities are embracing health care reform by joining together to create a new program called Tandem365; which allows older adults to age in place. Tandem365 is seeking clinical case management staff who have the ability and skills to help our participants navigate through the health care system. In this community based program, the MSW Navigator will be providing participant assessment, care, teaching and support, caregiver support, and assessment of needs in the home, doctor’s office, or a facility. The MSW Navigator works as a team with an RN Navigator and interdisciplinary team to develop plan of care, including supportive services necessary for promoting optimum health and decreasing hospitalization and emergency room care.   MSW interventions may include education, counseling, referrals, coaching, and therapeutic conversation to the Tandem365 participants in the home setting.

Job Qualifications

The ideal candidate will be well organized, self-motivated, and driven to help develop a new program while providing in-home care, education, and continued coordination of care to their participants. Should be comfortable caring for some individuals with behavioral health conditions in addition to complex and chronic medical conditions. Must have the ability to work well as a team as well as independently. Must have the ability to work within a multi-cultural community with varied  socioeconomic and psychosocial needs. Must have familiarity with community resources for connecting individuals to and working collaboratively with community agencies.
