Administrative Assistant - Intake

JOB SUMMARY:

Provides clerical support to assigned function(s).  Duties include, but are not limited to, typing, data entry of information.  Performs a variety of duties including receiving client referrals, data entry of referrals, and verifying insurance coverages.

Job Requirements:

Requires high school diploma or equivalent. Prior experience with patient scheduling and/or medical assistant background is valued. Strong computer skills are necessary as well as strong interpersonal skills to communicate effectively with all levels of staff as well as with various providers of care. Requires ability to handle difficult situations in a courteous, tactful, and respectful manner.

Job Details:

This position is part-time - 2 full days one week and 3 full days the next. Every other Wednesday and all day Thursday and Friday.  Hours are 9:00am - 5:30pm.

 



 

Bus Driver

Job Description: Transport residents to and from activities such as shopping, concerts, scenic drives, etc. Will also transport employees from off-site parking to Fulton Manor.

Job Requirements: Must have a CDL license. Relevant bus driving experience highly preferred.

Job Details: This position is per diem; main hours needed for coverage are Monday through Friday, 5:30am - 8:30am and/or 2:00pm - 5:30pm.  This position may cross train to cover some courier duties as well.

Floor Care Specialist

Job Summary
Provides scheduled floor maintenance, corrective and care of corridor and other assigned areas of the facility in accordance with housekeeping procedures, schedules and standards or performance.

Job Qualifications
Ability to lift and move furniture (a minimum of 50 pounds), work in small areas, bend and kneel easily and ability to use a ladder.  Must be a self-motivated person and be able to work independently.  Preferred experience with industrial carpet cleaning equipment.

Grounds Technician (part-time)

Job Description

Performs routine exterior maintenance to resident homes and other buildings on all 4 Holland Home campuses in the greater Grand Rapids area.

 

Job Qualifications

Requires knowledge normally acquired through a general high school diploma or equivalent. Requires 1-2 years of exterior maintenance experience.  Proof of Michigan driver's license, vehicle liability insurance, and reliable transportation are also required.

 

Job Details

This position is part-time working about 24 hours per week.

Helpers of Holland Home - Home Assistant

Responsibilities
Providing in home care for the elderly within the community through personal cares, light housekeeping, basic food prep, and transportation needs. Also includes assisting with ambulation and activities of daily living.

Skills required
Valid MI drivers license, reliable source of transportation, and current auto insurance. Experience working with the elderly is preferred but not required. Must have strong interpersonal skills and be organized and self motivated. Successful candidates have good computer skills and have access to email.

Other information
Prefer at least 4 hours of consecutive open availability or 8 hours of full shift availability.  Position will require 3 holidays a year and one week-end a month of on call responsibility.

Details
This position is at Helpers of Holland Home. It is a part time position during all shifts, and provides an average 15/20 hours per week may be more with individual availability.

Internship Summary

Internship - Human Resources / Central Scheduling

Internship Summary

Provide assistance to human resource staff and central scheduling department as needed.  Work closely with recruiting staff on coordinating interviews, conducting reference checks, assist with special projects, etc.

Hours

Varied - could be as early as 7:30AM and as late as 5:00PM.

LPN - Weekend On Call Rotation

The candidate would not be required to work every weekend, but would be added to a rotation with other Licensed Nurses and would fill in as back up as needed. Shift is primarily Friday 4:30 pm – 8:30 am the following Monday.   All after hours on call work is performed remotely.  This does not mean you must stay home.  You may be mobile, but you are required to respond within 30 minutes to all calls.

Job Summary

RN - Care Resources (P.A.C.E. program)

Job Description

Work in a clinic setting serving a geriatric population. Work under the supervision of a physician and serve as a key member of the interdisciplinary team.  Responsible for assessing, coordinating, monitoring, educating, and providing the clinical treatments to the frail elderly participants. Must be able to appropriately schedule and triage clinic visits for physician staff. Provide supplies and services required to meet the ongoing needs of the participants at the facility and/or in their home. Provide home safety environmental assessment at the participants home setting.

Job Requirements

Requires licensed RN in the State of Michigan. Must have a minimum of 1 year of nursing experience working with the geriatric population.  Preferred candidates possess recent emergency, urgent and acute care experience. Wound care skills or wililngness to be trained.  IV skills highly valued.

This position is full-time, working Monday through Friday days. Requires participation in the on-call rotation with 5-6 other RNs once oriented to the position.

RN Health Care Navigator - HHS, Health Options

Job Description

Do you want to be on the leading edge of health care?   Holland Home, Clark Retirement, Life EMS, Porter Hills, and Sunset Retirement Communities are embracing health care reform by joining together to create a new program called Tandem365; which allows older adults to age in place. Tandem365 is seeking clinical case management staff who have the ability and skills to help our participants navigate through the health care system . This is a community based program, where the RN Navigator will be providing participant support, caregiver support, and assessment of needs in the home, doctor’s office, or a facility.

Job Qualifications

This position requires an RN license in the State of Michigan. The ideal candidate will be well organized, self-motivated, and driven to help develop a new program while providing in-home care, education, and continued coordination of care to their participants. Must have the ability to work well as a team as well as independently. Minimum of 2 years of acute care and/or certified home care experience required. Prefer someone with prior experience in community based services providing case management.

RN Hospice Staff Nurse - per diem

Job Description

Assist during high census periods, covering for full-time team members who are on vacation or leave, and assisting with special projects for hospice. This position is per diem, working at least once a month and then filling in as needed to assist the hospice team.  Hours are not guaranteed.

RN Liaison

Job Responsibilities

Coordinates referrals and admissions from area hospitals and skilled nursing facilities to Holland Home services and facilities. Duties include meeting with Holland Home residents and prospective clients/patients, assessing individual’s current status and readiness for Holland Home services, completing required paperwork and working with internal and external parties to facilitate the admission. Refers individuals to other programs as appropriate.

Job Qualifications

Requires licensed RN in the State of Michigan. Requires hospital, certified home care, skilled nursing, or hospice experience. Requires strong computer skills. Excellent communication skills and relationship building is highly valued.
 

Job Details: Position is for 32 hours per week including a weekend rotation which includes covering for 4 hour shifts on Saturday and Sunday in the hospital to assist with admission inquiries and coordinating safe and appropriate discharges from the hospital to within the Holland Home system.  This is considered a full-time position with benefits, paid time off, and retirement savings plan.

RN Psych Case Manager - home care, per diem

Job Responsibilities

Assess client’s health status, establish nursing diagnoses, develop and implement nursing care plan under the orders of the supervising physician. Maintain communications with clients, caregivers, interdisciplinary home care team members and physicians to enable the client to achieve the highest level of function possible.

Job Requirements

Michigan licensure as an RN required. BSN preferred. Per federal mandates, minimum of 2 years experience in an in-patient or community mental health setting. ANA board certification strongly preferred. Prior experience in community health and/or home care preferred. Must have reliable means of transportation.

RN Triage Nurse (Hospice)

Job Description

Provide assessment, educational, and evaluative services over the phone to meet the needs of terminally ill patients as well as their families and caregivers. Respond to telephone requests for assistance by addressing the request over the telephone or by coordinating a patient visit with the RN Extended Care team.

Job Requirements

Requires RN licensure in the State of Michigan. Requires a minimum of 2 years of med/surg or skilled nursing experience.  Prior hospice experience is strongly desired.

Job Details

This position averages 30 hours per week, working every other weekend for 10 hours each weekend day (8am - 6pm) and 8 hours for 2 weekdays one week and 8 hours for 3 weekdays the other week (Noon - 8pm).  The position is based at the Corporate Office and will work from the Corporate Office. 

Security/Maintenance Assistant

Job Summary

Patrols all Holland Home facilities and grounds during 3rd shift. Maintains visibility to deter unauthorized activities.

Job Qualifications

Requires knowledge normally acquired through a general high school diploma or equivalent. Prior experience in security and some maintenance experience is highly valued. Must have valid driver's license, proof of insurance, and clean driving record.

Job Details

This position is per diem, meaning as needed to cover call-ins and vacations for current staff. Position is 3rd shift and weekends.

Social Services - Bachelor’s prepared Social Worker, requires licensure with the State of Michigan

Job Description

Provide support to residents and their families by offering emotional encouragement, information, education, and long-range planning. Conducts assessments of social and emotional factors and develops a care plan for assigned Residents.  Implements care plan intervention providing counseling, crisis intervention and emotional support to residents and their families.

Job Requirements

Requires a Bachelor's or Master's degree in Social Work.  Fully licensed or working towards licensure in the State of Michigan is required. Requires minimum of 1 year of social work experience in a long term care or community based setting.  Knowledge of community based resources is preferred.

Social Work (MSW) Health Care Navigator - HHS, Health Options

The ideal candidate will be well organized, self-motivated, and driven to help develop a new program while providing in-home care, education, and continued coordination of care to their participants. Should be comfortable caring for some individuals with behavioral health conditions in addition to complex and chronic medical conditions. Must have the ability to work well as a team as well as independently. Must have the ability to work within a multi-cultural community with varied  socioeconomic and psychosocial needs. Must have familiarity with community resources for connecting individuals to and working collaboratively with community agencies. Must have the ability to work independently and problem solve in a timely manner, present and participate in daily interdisciplinary team meetings, maintain ability to be flexible to work assignments, be comfortable in meeting participants in their home for enrollment, assessments, and routine visits.

Speech Therapist - home care

Job Responsibilities

Diagnoses and treats speech, language, swallowing and cognitive disorders.  Home care territory covered is generally Kent County and just into the outskirts of surrounding counties.

Job Requirements

Requires Certificate of Clinical Competence (CCC) from the American Speech and Hearing Association. Requires a minimum of 2 years of speech/pathology experience. Geriatric and/or home care experience is preferred. Must have proof of valid driver's license, vehicle liability insurance, and reliable transportation to make home visits.
