HHS Health Options is a resource for elders, caregivers and others with long-term care needs. Through a variety of activities, case managers coordinate the services that help with daily life and assure the appropriate level of gentle and dignified care for each individual's need.

Care Resources
Care Resources is a Program of All-inclusive Care for the Elderly (PACE), a long-term comprehensive health care program that enables frail elderly adults to live at home in their communities for as long as medically and socially possible. Care Resources is committed to helping frail elderly adults maintain their independence, dignity and quality of life. Care Resources is a Health Plan which provides all medical care (24-hours a day), a Day Health Center, home care services and transportation.
